Care Management Team

The care management team is a peer navigation team that will assist guests post-discharge with ongoing peer support services and provide linkage for guests to outpatient services and appropriate community resources.

The Bridge Center has many community resources to assist guests who are discharged in accessing an appropriate level of care throughout the community. As part of the discharge planning process, all guests are scheduled for an outpatient provider appointment, as well as a physical health appointment, and given additional resource information.

If someone is discharged without permanent, stable housing, as part of the discharge planning process, the Bridge Center will assist the individual in securing temporary housing, such as a residential treatment facility, group home, or shelter.
