After Crisis Care: What Happens Next
Support doesn’t end at discharge. Crisis care is an important first step — but recovery continues after stabilization. At the Bridge Center for Hope, discharge planning is a thoughtful process designed to ensure that every guest leaves with support, connection, and a clear path forward.
The goal is not simply to discharge someone, but to help them transition safely into the next stage of care and recovery.
Before discharge, the care team works closely with each guest to prepare for what comes next. This planning includes scheduling important follow-up care and connecting guests with resources that support continued healing. Before leaving the Bridge Center, all guests are:
Scheduled for an outpatient behavioral health provider appointment
Scheduled for a physical health appointment
Provided with additional resource information tailored to their needs
These steps help ensure continuity of care and reduce the risk of returning to crisis.
Family members and natural supports are encouraged to be involved in the treatment and discharge process when it aligns with the guest’s treatment plan — and only with the guest’s written consent.
Natural supports are defined as personal relationships developed in the community that enhance safety and quality of life, including family members, friends, and trusted individuals. When appropriate, involving these supports can strengthen recovery and provide ongoing encouragement after discharge.
Housing stability is an important part of recovery. When a guest does not have permanent, stable housing, the Bridge Center assists during the discharge planning process to help secure temporary housing options.
This may include referrals to:
Residential treatment facilities
Group homes
Shelters
The goal is to ensure that guests leave the Bridge Center with a safe place to go and access to continued support.
The Bridge Center works closely with a wide network of community-based providers and resources. Guests are discharged to the most appropriate level of care based on their individual needs.
This may include outpatient services, substance use treatment, medical care, or other community supports. Discharge planning is individualized, with the aim of helping each guest move forward with the right care at the right time.
In some cases, a guest may be transferred to another treatment facility instead of being discharged home. This typically happens when the individual develops urgent or emergent medical complications that require a higher level of medical care.
These decisions are made to prioritize safety and ensure that guests receive the care best suited to their needs.
The Bridge Center for Hope exists to support people through moments of crisis — and beyond. Discharge is not an ending, but a transition to continued care, stability, and recovery.
